Salesforce had new profiles that needed to be synced with Okta. They weren't syncing because the OAUTH app had been deleted from the Salesforce environment. After adding the app, I had to update the Consumer Key and Secret in Okta with the new one from the OAUTH app. When I re-authenticated, the settings for provisioning were cleared. Why did that happen?

This might have happened because you used a new application in SFDC, but normally the settings should not be cleared and they should stay the same. If you can replicate this issue at will, I would recommend to reach out to Support for additional investigation.
