Added a Rule in Password Authenticator for Self Service Password Reset and turned on all three options ( Email , SMS & Okta Verify) for both Forgot Password and Unlock Account.
When I go to login page, and clicked "unlock account", all three recovery options are displayed. However, when I clicked "Forgot Password" , only email option is given. SMS and Okta Verify options are not displayed.

Hi! Under Security > Authentication > Password, ensure that under Account Recovery both SMS and email is checked, this is enabled before the rule. They are however, unique to the policy. So it may depend on the policy that is applying (Active Directory, Okta, group, etc.) in your unique case.
Within the rule, you will specify whether users are allowed to Change/Reset/Unlock their accounts in a self-service fashion and the rules surrounding those specifications. You can have multiple rules in each policy, in addition to multiple policies. So it may be helpful to look at your system log to determine which policy is being applied.
