New to Okta. Can group rules that are newly created run against already existing users or is it only designed for newly onboarded users? Are there consequences to running them in an automated fashion? What would be an example of a trigger? Thanks for any and all input.

Yes but you need to base the membership on something, i.e. email address, department, title, etc.
In general, group rules run against all users in your org, except those you explicitly exclude. And once a rule is activated, it works continuously, with no need to trigger them manually. You configure the IF condition(s), then the rule will dynamically add any user who meets the criteria you defined.
