    Hi @RonH.73785 (Customer)​ , Thank you for reaching out to the Okta Community! 

     

    The most common cause for this would be a misconfiguration of the password polices (in particular number of lock-out attempts).  

    I recommend checking that there is a parity between the Okta policy and the AD one. 

    Another cause could be failed MFA attempts (currently hardcoded at 5 failures allowed before lockout). 

    That being said, the cases might have to be investigated individually. 

    I recommend checking the Okta System Logs for the events leading up to the lockouts.
