We are attempting to setup provisioning of Salesforce from Okta. I have the SAML connection working.
When I attempt to re-authenticate with Salesforce, I show I get a login to Salesforce but I get the following in Okta:

This error can be caused mostly by incorrect permissions set for the app, but also could be because:
