User is not being challenged for 2FA

I have set up the following factor enrollment:

My account is part of the 12345 group and is using the Test application specified in the rule.

 

When I logged in for the first time, I had to set up 2FA using Google Authenticator.

However, on subsequent logins, I'm not challenged for a 2FA code.

 

Why is 2FA not working?

    Enrollment is the first step. You also need a sign-on policy that will prompt the user for MFA. Do you have that?
