okta verify immediately shows error after tapping Yes, It's me

We are doing MFA with RADIUS Agent, some users have issue that once they get the notification on the phone (okta verify app), they tap on Yes, it's me and immediately get an error message 'Unable to send push authentication'.

 

We've tried remove the account and enroll again as well as re-install the app, but nothing helps.

Logs only show the last message that "A push was sent to a user for verification, success" and that's it as there was no response back.

 

The users devices are android phones.

 

Please advise.

    Please have your users check the following:

    Okta Verify notifications are turned on.

    The date and time on your device are set to automatic. The clock on your device must be in sync with the time maintained by your cellular provider’s network. Small differences between your device clock and the network clock can cause issues.

    The latest version of Okta Verify is installed on your device. For details, see Set up Okta Verify on Android devices.
