Error while connect to Salesforce using Federation ID

I have created SSO app in Salesforce and configured SAML configuration (Login URL) in Okta, When I click on Enable API integration on Provisioning tab then I got the following error:

Please review the form to correct the following error(s):

Could not verify the Salesforce administrator login and password; please confirm that these are set correctly

Whereas in Salesforce Login History it says Authentication Success, and the SAML Assertion Validator gives the following error:

Subject: 

Unable to map the subject to a Salesforce.com user

How do I solve the above issue?

    Thank you for posting on the Okta Help center.

     

    Make sure you follow the steps found here: https://saml-doc.okta.com/Provisioning_Docs/Salesforce_Provisioning and to append the token Salesforce provided to you to your password, no spaces or other characters as in : passwordXXXXXXXXXXXXXXXX,

    where XXXXXXXXXXXXXXXX is the API Token.

    failure : API_DISABLED_FOR_ORG.
