Or does the password only sync the next time a user changes their Okta password?

Hi Brian,
Thank you for contacting Okta Community! My name is Daisy and I will be assisting you with this case. When the "Sync Password" - Sync Okta Password option is enabled, the Okta password will be synced to G suite immediately. Whenever you change your password in Okta, the new password is pushed to G Suite.
Note: Okta password policy should match Google's requirements in order for provisioning to work.
