I have a Pixel 9a and tried to add a credential by scanning a QR code this morning. I had not granted the Okta app camera permissions, so I selected the "ask every time" option. This did not allow me to scan the code. I had to choose (temporarily, of course) the "allow when using the app" level of permission. The app should work with the "ask every time" level of permission. I am not able to directly contact Okta about this bug in their app. Are they aware of it?

If you believe this is a bug this needs to be reported though the proper channels. In this case we recommend to reach out to your Okta Administrator and have them open a case with Support to further investigate this issue.
