The use case in question is asking if PFSense OpenVPN can send a user authentication request to a Radius server that uses Okta to do MFA? If so, is there a reference that explains how that might work?

I have not been able to find any resource that suggests this is supported out-of-the-box, but you might be able to configure it via the Generic RADIUS App integration (Okta Admin Dashboard → Applications → Browse App Catalog → search for "RADIUS app". ). I'll post the documentation below:
