If I set up AAD, I should be able to leverage Okta for Azure MFA, but I don't think I can do that with Microsoft 365 integration...However, if I set up AAD can I manage Office App assignments or do I also need Microsoft 365?

You can use Office with AAD, and as provisioning options you can use Licenses and Roles Management Only and Profile Sync. Please see our doc below as well:
