I took a okta admin exam. i am not able to set a authentication policy for an app with passworrd and security factors. when i create a rule for password + another factor. Its shows only okta verify not security questions

you should add security question in authenticcator,

The Security question cannot be used for app level sign on policies because it’s also a “Knowledge” type Authenticator like the Password. When setting up a policy, you would need to user 2 different types. Example in the screenshot below:

The Security Question authenticator can be used as step-up or additional verification when password is used as the primary factor, specifically, if the primary factor in the user's global session policy is A password. Okta recommends against using Security Questions in any authentication flow.
