Deactivated and reactivated user in Okta admin portal
No duplicates or conflicts with user account found
Activity shows good and sign on attempts successful

End user has gone through setup for 2FA and text verifications

Use a free browser plugin/tool like SAML-tracer and see if the results provide any insight. Check logs in calling SP app as to me the error sounds like the landing page is having the issue not Okta.
