From an earlier thread: Go to Okta dashboard -> settings -> extra verifications. Remove "Okta Verify". The remove button changes to "Set up" which will get you to a new QR code.
@yoazh (yoazh) Do you mean that you can't active okta verify succesful by scan the QR code? Is there any error message? Does the time of your mobile phone sync with network? Does your Okta verify with latest version? If still can't active, please let your admin create case with Okta support team.
