I cannot seem to get a static value set in username/password/extrafieldvalue to be populated in the website form nor will it try to submit the website form automatically.
I've created a "Template Plugin App 3 Fields" SWA app for the site https://www.advantageengagement.com/1063/login_company.php. This site uses a shared value "Company Code" so I set CSS selector *company_username for the username and password and extra field value in the SWA app config and hardcoded the same value for both after indicating that they are set by admin. I also set the "Login button selector" to *submit-login and check to box to automatically login. I have the redirect URL set to https://www.advantageengagement.com/p_index.php.
I found this post that makes it sound like I'm on the right track but again I cannot seem to get the "Company Code" populated nor auto submit the form. https://support.okta.com/help/s/article/How-to-configure-single-field-SWA-app?language=en_US
I've tried to create a plain "SWA - Secure Web Authentication" and a "Template Two Page Plugin App" SWA apps as well just to see if they would work but have the same trouble.

I'm not seeing any template app that would fit for this site. Neither "three fields" nor the "two page" templates would work here. You would have needed a "three fields not on the same page" type of template here (good idea for feature request?).
In this situation, I would have to mention what I said here as well.
"If you can't get it to work, you might want to reach out to your Okta Account Executive or Custom Success Manager and they can submit a request on behalf of you to have the app integrated if it meets all requirements.
That being said, there would be no SLA as the deployments would be based on the App Integration's team backlog."
