We have a site integrated into Okta when they log in but one user is getting 401 Unauthorized error. When we checked the logs in Okta, all of the log in attempts show "SUCCESS". Any ideas on the potential cause of this? We have confirmed that the email in AD and Okta are the same.

If this is a SAML application, then the system logs will not help that much. If the user is getting that error the problem could be that he is not assigned to the application. If he is, maybe his account is not provisioned on the application side or he does not have an account.
I would recommend to review the access on the application side and logs to have additional inside on this matter.
