Seems like you are giving wrong credentials , can you please once verify this.

The client is network switch , and set the primary and secondary okta verify server, and failed to authenticate via main okta service but the secondary is fine. so can you help to find why.

In that case I need more insight on how primary and secondary is configured. But as a primary step you can verify whether the preshared key (Secret Key) matches with okta tenant.
