We are migrating off of DUO to Okta Verify. I have one application where DUO prompts and then Okta Verify prompts and the app gets stuck in a loop. I think this is a rule or policy issue but I don't have this problem on our other applications. Any ideas?

Is it possible that the application itself is prompting for MFA? That might explain being prompted twice, once by the app and again by Okta. It seems unlikely but not unheard of.
