I found a couple of external threads that indicate this might be a configuration issue:

I do want to note that the SimpleSAMLphp library is not officially supported by Okta (only the SDKs that we have created are) and that this issue points to more of a library or configuration issue, so I would recommend looking for assistance from the owners of this library going forward.
