Failed to enroll in push authentication

After scanning the barcode to enroll with my iphone I get the account added but a popup saying "Failed to enroll in push authentication. You can still manually enter the authentication code" with a "retry" option.

 

When I hit retry another duplicate account is added and the error pops up again.

 

Authentication doesn't work

 

Trying to manually enter the auth code also doesn't work.

    You may want to check and make sure the time on your iPhone is set to Automatic (set by your carrier). Also, make sure your internet connection is stable. Let me know if this is still happening after checking this.
