Can I add opt-in fields to new account creation form?

Seeking to add opt-in fields on a new account creation form - can this be done? Would provide guidance like this: May [company] provide you with personalized communications about [company] and partner products, services, offers and events? EMAIL=YES and/or PHONE=YES

 

If not, can an Eloqua form post new user data into OKTA, for new account creation? Then OKTA assigns the application to the user and activates them?
