    If you have Self Service enabled in you org/tenant the user might have added the app by himself and not actually get assigned to the application, you can check if Self Service is enabled by going to your admin dashboard > Applications > Self Service, under User App Requests you see three options, to better understand what each option does read the informative text from the right of the page.
