Cybsafe Provisioning "Errors reported by remote server: Operation is not permitted based on the supplied authorization"

Overview
Cybsafe provisioning flow fails with the following error visible in the Okta dashboard:
Automatic provisioning of user <user> to app Cybsafe failed: Error while verifying if user <user> exists: Forbidden. Errors reported by remote server: Operation is not permitted based on the supplied authorization.

Cause
Solution
- Go to Okta Admin Console and navigate to Applications > Applications > Cybsafe > Provisioning > Integration > Edit.
- Enter the API Token received from Cybsafe in the API Token field.
- Click Test API Credentials, verify the connection was successful, and then click Save.
- Attempt the failed tasks again. Navigate to Dashboard > Tasks. Any failed assignments should appear under Tasks.
- After locating the failed task for the user that needs to be retried, click Retry Selected.
