    If the issue is affecting all users, you will need to review the app configuration and authentication policies.  

    If it's just one user affected, you will need to compare their authentication context as opposed to the working users.  

    Start by checking the Okta System Logs for specifics around the failure. Based on the provided screenshot, it might be that they did not fulfill the requirements imposed by the app authentication policy

    Another thing to look for is the user application assignment to make sure the provisioning task did not fail in some way. Check your Okta Admin Dashboard for any "Application assignments encountered errors" tasks.  

    The latter is less likely though as the error would typically say something along the line of "you have not been assigned to this app".

    Hi Wahid: The error you're getting shows the icon for the Okta verify app. That tells me that you're getting blocked by a policy, rather than by the app integration settings. Check:

    1. Global session policies and rules
    2. Account management policies and rules
    3. App authentication policies and rules.

    Check your account to ensure that you meet the requirements of the policies and rules that apply to you. Check the system log. Since rules have names, you can identify the rule that you are failing against in the log.
