Can't figure out why SCIM app stopped provisioning users (Okta Integrator Edition)

We're an app developer with a functioning Okta integration via SAML/SCIM, and many customers live using that integration. We tested this integration successfully using our own free Okta tenant (Okta Integrator Free plan, migrated from Okta Developer Edition). However, we've noticed sometime in the last couple of months SCIM provisioning stopped working from our demo Okta tenant.

 

Specifically, although we have the app configured correctly in Okta and users are assigned to it properly, no SCIM provisioning requests are appearing in the logs on either side (Okta or application) - it appears Okta just isn't attempting any provisioning when users are assigned our SAML+SCIM application. I have already checked and confirmed:

 

  1. In General, Provisioning is set to SCIM
  2. Provisioning -> To App has Create Users, Update User Attributes, and Deactivate Users enabled
  3. Provisioning -> Integration -> SCIM Connection has "Supported provisioning actions" including Push New Users, Push Profile Updates, and Push Groups
  4. SAML Name ID Format is set to Email Address, and if I click in on my test user, the Application Assignment Username is set properly to my email address
  5. The "Test Connector Configuration" button works and shows "Connector configured successfully"
  6. There is a mapping from Okta User --> App User in the Profile Editor.
  7. My test user has status Active, and no error messages are displaying.
  8. There are no provisioning actions, or "provisioning skipped" logs, in the System Log.

 

What else could be going wrong?
