I was reviewing the OIG API documentation laid out here https://developer.okta.com/docs/api/iga/
We're attempting to figure out a way to CREATE OIG Policy Rules via API, but it looks like we're only able to GRANT.
Has anyone been able to successfully create policy rules via API or is it only possible manually?

Generally speaking, not all Okta GUI options and features are available via API. Some are made gradually available and some are never exposed for public use.
As far as I can see, this is marked as Beta, so I'm not expecting all options to be available, but I recommend opening a case to confirm with our dedicated OIG Support Team.
