Is it possible to customize the end user help form

Is it possible to customize the end-user "request help" form on the Sign-in help page?

Hi @JoshB.17590 (Customer) , Thank you for reaching out to the Okta Community!
The end-user help request page is not customizable currently but you can change the URL for the link and perhaps add a custom/self-hosted solution.
To change the link go to Admin Dashboard → Customization → Brands → <brand name> → Pages → Sign-in Page → Labels.
