To get the QR code ,you have to reset the okta verify from your account for that you must be able to access the Okta End-User Dashboard using a security method other than Okta Verify, such as SMS (text), email, etc. .If no other MFA factor is enabled to your org you have to contact your administrator to reset the factor.
