After creating a new application for SAML integration, under Applications > app name > Sign On > View SAML setup instructions, no metadata URL is present. Where can this URL be found / why might it be missing from the setup page?

I would open a ticket for that. I can't think of any reason it wouldn't give you the metadata URL.
Found it on the Sign On tab. Need to click Actions for the generated certificate and choose "View IdP Metadata".
There should also be a View SAML Setup Instructions link on the right side of the page when you're on the Sign On tab. That will open a new tab containing the individual metadata components, as well as the full metadata in a box at the bottom of the page, which you can then copy and paste elsewhere.
