When you install Okta Verify on a mobile device and enroll it in your Okta org, that registration is specific to that device and that instance of Okta Verify. This is a security feature, to prevent someone else from accessing your Okta account in the event of your device being compromised, stolen, or even cloned.

If you replace that device and install Okta Verify, the new device must be enrolled again. And, unfortunately, if Okta Verify is your only enrolled MFA factor, you will have to get an administrator of your Okta org to clear (delete) your old Okta Verify enrollment before you will be able to enroll the new device.

The same is true if you make any change to the device that affects the installation of Okta Verify, such as uninstalling/reinstalling Okta Verify, or restoring your device from a backup.
