We are using F5 APM in front of some custom web apps & the setup doc tells me that it only supports SP-initiated flows. I have created a bookmark to simulate IdP initiated.
How can I do user provisioning in this situation?

Can you not use the same group assignments for the app and bookmark?
If the app integration setup doesn't work from IdP initiated flow for SSO, I assume it can't provision to the app.

I believe this can be accomplished with Group Assignments and having a duplicate app instance. See: https://help.okta.com/en/prod/Content/Topics/Provisioning/lcm/lcm-create-duplicate-app-instance.htm
