I tried searching through the community, but couldn't find anything specific to this situation. I apologize if it's a repeat.
We have a third-party application which we assign to users using the "Administrator sets username and password" setting. When viewing an individual's application settings, there is a message indicating whether a password has been set for that user/application. Is there a report I can run that will tell me which users are assigned to a particular application without a password set? I have noticed a column titled "Last Password Change" on various reports, but I've found examples where that's blank, yet a password is still set.

Hi Kyle. All users in Okta have to have a password set, unless they are in created at a 'Federated' user, which is another use case. So any active accounts have a password.
The Last Password Change is just that. It's a timestamp of when the password was changed. Not the timestamp of the original password that was set when the account was created/activated.
Best thing to answer your question is to get hold of Rockstar. Set up accounts in the various states that you are interested in, and then use Rockstar to export them including all the metadata attributes and see if you can dig out the information you need from that.
