We are getting this error when trying to do an LDAP authentication with Okta, any ideas?

It looks like the account you're using may not have the proper permissions in Okta. What admin role is assigned to the account you're using to authenticate? If you aren't using one already, I would try testing again with an account that is confirmed to be a Super Admin.
Can Okta "Black List" certain IP Addresses or URLS?
We elevated the user to Super Admin, same issue.
Check under Directory -> Directory Integrations -> Inactive
you may just need to re-enable it
