Is there any documentation that we have around the formatting required for each of the available countries that Okta supports? For example, we are currently running into issues with our German users not being able to set up SMS or Voice Call Verification due to the country using different phone number formats.

Unfortunately I don't see any such documentation, so I suggest creating a case with OKta Support so they can help advise you. During that process we can either create a support KB article, or help ensure that our product documentation be updated.
