We have a user who has set up the voice option as his MFA. The call is received during initial setup, but afterwards the call is never placed. I can not find anywhere in the log where the call is being initiated.

I suggest creating a support case in order to troubleshoot this issue. We may need to take a look at how your Sign On Policies and Rules are configured.
