Entity ID / Issuer mismatch in SAML response from Okta

We are an SP supporting Okta as an IDP. Experiencing an issue where in the post redirect SAML response we are receiving from Okta, the entity ID seems to be a different, valid, app that does not match up with the entity ID provided in the initial SAML request / metadata.

 

Has anyone seen this before or have any ideas why the entity ID / issuer would be different in the SAML response relative to the request that was made?
