A custom SAML app in Okta will have a single ACS URL that you can configure. (The configuration item will actually be for the "SSO URL" in Okta. Same thing.) So if you wanted to have multiple SSO URL's, you would need to configure additional SAML apps. Make them identical, except for the SSO URL.

The SSO URL will be used for both IdP-initiated and SP-initiated logins. However, you can configure an index of several SSO URL's within a single custom SAML app in Okta. The main SSO URL will still be used for IdP-initiated logins, but an SP-initiated login can specify what SSO URL to use for Okta's SAML Response.

Go to your Okta admin console and pull up your custom SAML app. Go to the "General" tab and then click the "Edit" button for "SAML Settings", then click "Next" to get to the SAML configuration page. This is where you'll find the SSO URL. Right below that will be an option for "Allow this app to request other SSO URLs". Put a check mark next to this, and an additional configuration item will appear where you can now enter those additional SSO URL's.
