Application sign on not successful

We have an application setup to use " Users share a single username and password set by administrator".  Sign on to the app via Okta is inconsistent. The Okta log for the app says sign on is always successful, but in reality sometimes the browser is just sits at the application login page. This is a new occurrence. We have had the application configured this way in Okta for a long time, but the failures just recently started occurring.
