I'm trying to create an app with web auth for sign in but it has 3 fields

Yes. Navigate to Applications > Add Application > search "Template Plugin App 3 Fields" > Add. You'll be given the option to configure three fields. The third field must be static for all users, and all three fields must be present on the same login form (no two-step authentication).
What Gabriel sais is correct, but if you do have more users with different 2rd field values (and its splitable in groups) than you could create multiple apps and assign the correct one to the correct group. giving a little bit more flexibility to setting it up.

2nd is username so that is going to be different and
3rd is the password so different as well
