We have a user that is getting an access denied prompt but it still allows them to log in. Also, the credentials will not save and they have to input password every time. The user logs in multiple times a day. Is there a way to fix this? We've tried multiple browsers and remembering sign in.

Hello Bret,
This issue need a deeper investigation. We will need to check if the user is triggering a policy, if he does why the policy does not apply. Or if this issue is cause by Okta or environmental.
As next step, we recommend opening a case with Support for a better understanding of the issue and further troubleshooting steps.
