Okta Privileged Access Request Approvals Fails With "You provided invalid inputs" or "Unauthorized" Error

Overview
When an Access Request is approved by an authorized approver in the Access Requests Inbox, the request status displays as 'Failed'. The Okta Privileged Access (OPA) connector for Access Requests fails with any of the following error messages:
You provided invalid inputs. Please try again.
Failed to run task: approval Action (Unauthorized)

Cause
The issue is caused by an incorrect Okta Privileged Access (OPA) Team Name entered during the configuration of the Privileged Access Request Connection. The connector fails if the configured OPA Team Name is incorrect or includes leading or trailing spaces. This condition can be verified in the history details for the request within the Access Requests logs.
Solution
To resolve this issue, correct the team name in the connector configuration. The team name under "Teams" may need to be corrected as well. The team name must be the exact OPA team name.
- Go to the configuration settings for the Privileged Access Request Connection and click Edit connection.
- Disable the connection and then confirm the Disable action.
- Once disabled, the Edit Connection box will change to Connect. Select this to reconfigure the connection.
- Locate the OPA Team Name field.
- Verify that no leading or trailing spaces are included in the field.
- Click Update connection to save the new settings.
- Initiate a new access request in OPA to test the approval process.
