This article discusses why new users are not prompted to select a security image at initial login.

Solution

Here are the circumstances under which a security image appears/does not appear:

  1. When an account is created and initially placed in STAGED status, then later set to ACTIVE, the user will need to manually set a security image in order to complete the account setup.
  2. When an account is created and immediately set to ACTIVE (without being placed in STAGED status first), Okta will randomly select/auto-generate a security image for the user account. The user will not receive any prompts to set a security image since this has already been set by Okta.
  3. The org was upgraded to the Okta Identity Engine (OIE). For OIE, the security image was deprecated.
