GitHub Provisioning Error "The URL does not match the required pattern"

Overview
When enabling the provisioning connector for the GitHub integration, an error appears in the Okta dashboard:
The URL does not match the required pattern.

Solution
The GitHub URL field must be populated with the full URL of the GitHub Enterprise Cloud server instance. Currently, on-prem instances of GitHub do not support SCIM API. The GitHub Enterprise server app only works with cloud instances.
GitHub Enterprise Server is software deployed on a virtual machine (on-premise here is a bit of a misnomer since the VM could be in AWS). GitHub Enterprise Cloud, on the other hand, is an enterprise-level service at GitHub.com.
