This article explains why the security image would not appear in an Okta Classic Embedded Sign-In Widget.

Solution
Security Image is designed to be used by the Okta-hosted Sign-In Widget and is not supported in an embedded widget.
If the admin has enabled a security image, there will be a redirect to the Okta Welcome Page, where end users will be prompted to select a security image and a "forgot password" security question.
