Hi support,
I am trying to walk one of my German employees through their Okta configuration. We're able to get as far as selecting the SMS authentication option, but as soon as he enters the code sent to his phone we get the following error:
Not found: Resource not found: mbl4meib2tU2efl8R357 (UserFactor)
I seem him listed as active in Okta, and everything with his Okta account appears to be OK. I didn't see any info regarding this specific error so I'm not sure where to start troubleshooting...

reset the mfa and try (or) as part of the mfa reset try by give your phone number and see what is behavior
I would start with the basics - put him in any MFA bypass group or remove his account from any MFA requirement to ensure he can login. If successful, check the Sign-on policy under Security--> Authentication and the MFA policy under Security--> Multifactor (Factory Types and Factor enrollment). Any screenshots would help.
