Use Three Attributes in Secure Web Authentication

I want to add an app using Secure Web Authentication (form based) and need to pass over three attributes - Company ID, User ID and Password? Has anyone encountered this and know of a workaround?

You would have to configure a template app.  Check out the link below.  If you have any questions on the process let me know.

Thanks Jeff! This is awesome. I am new to administering OKTA, and was previously using OneLogin so am still learning as I add apps to OKTA.
