I have seen this spicifically with customers that are using deligated authentication to AD. The password policy is not always the same as Okta's for an Okta mastered user. One of the things you can use to test this is have the user attempt to change the password in AD, and you should see the error code or the Group Policy preventing this. If you continue to have problems kindly file a support case and we can help you get to the bottom of it.
