Hi Teena,

We'll need more information to accurately identify the cause, but I'll take an educated guess.  If you are using Okta for federation with Office 365, when the user's password changes it must be manually updated on any thick clients or mobile devices (if not using Okta's OMM offering).  This will include mail accounts on mobile phones and tablets, Outlook thick client, Skype thick client, and if you're using a Mac even the "Reminders" application syncs with O365.
