Bank of America - Works verified app doesn't seem to support necessary fields

I am trying to get the Bank of America - Works verified app to, well, work. The problem seems to be that there are three dynamic fields on the login screen, and only two fields in the app. I have also attempted to create an app using the 3-field template, but the third field is not a static value; it's a user-inputted value. Has anyone had this issue, and if so, how did you resolve it?

The   3-field template app is not created to handle a dynamic fied. The last fild should be static and always the same in order for the template app to work.
